Introduction

Crestor (rosuvastatin) is a non-steroidal medication used in the treatment of hypercholesterolaemia. In patients with homozygous familial hypercholesterolaemia, the pharmacodynamics of rosuvastatin is unknown, but several reports suggest that it can reduce the rate of progression of heterozygous familial hypercholesterolaemia. To understand the mechanism of action of rosuvastatin, we investigated rosuvastatin's pharmacodynamics and associated clinical variables in a large population of homozygous familial hypercholesterolaemia. Rosuvastatin is a lipid-lowering medication that has been used to treat heterozygous familial hypercholesterolaemia. To date, rosuvastatin has been used to treat homozygous familial hypercholesterolaemia, as well as to reduce the rate of progression of heterozygous familial hypercholesterolaemia. The mechanism of action of rosuvastatin has been investigated in a variety of cell types, including human breast-cell line MCF-7. It was demonstrated that the inhibition of HMG-CoA reductase with rosuvastatin enhances the synthesis of androstenedione, a hormone involved in lipid metabolism. These findings suggest that rosuvastatin could be used as an alternative treatment for homozygous familial hypercholesterolaemia. In this study, we evaluated rosuvastatin pharmacodynamics and clinical variables in a large patient population. We also evaluated rosuvastatin pharmacodynamics and clinical variables in a different patient population. This study aims to describe rosuvastatin pharmacodynamics and clinical variables in a patient population.

Methods

Patients

A total of 461 patients with homozygous familial hypercholesterolaemia were included in the study. All patients underwent diagnostic evaluation of HFrEF with at least 1 of the following criteria: at least two of the following criteria: normal fasting glucose (or hemoglobin A1C) <70 mmol/L; or at least one of the following criteria: fasting plasma glucose (or hemoglobin A1C) >120 mmol/L; or at least one of the following criteria: fasting triglyceride (or total-triglycerides) (or total-isopropyl-toluamide-binding globulin) (or fasting plasma levels of low-density lipoprotein-Lipid; or levels of total-triglycerides, and/or levels of total-isopropyl-toluamide-binding globulin) (for the fasting variables, we used the values obtained from the American Heart Association (AHA) Adult Hyperlipidaemia Panel).

The diagnosis of homozygous familial hypercholesterolaemia was based on the criteria of the original report by Lai et al. (1997). Patients were divided into two groups according to their HFrEF: Group A: Group B: Group A + rosuvastatin; Group B + rosuvastatin. Patients in each of the two groups received the recommended starting dose of rosuvastatin (400 mg) and then increased daily, starting rosuvastatin from 200 mg (group A). Each group was tested for each of the HFrEF criteria and each of the criteria was divided into three groups (group A + rosuvastatin: 10 mg, 20 mg, and 40 mg, respectively; group A + rosuvastatin: 40 mg, 10 mg, and 20 mg, respectively; rosuvastatin: 10 mg, 20 mg, and 40 mg): Group A + rosuvastatin: 10 mg; rosuvastatin: 20 mg and 40 mg; the initial dose of rosuvastatin was 40 mg (group A + rosuvastatin: 10 mg).

Patients who were found to be clinically significant homozygous familial hypercholesterolaemia were considered to be clinically significant heterozygous familial hypercholesterolaemia and were considered to be clinically significant homozygous familial hypercholesterolaemia (HHF) if they were:

• HHF;

• HFrEF: the mean follow-up time was 14.5 months for patients with HHF and 13.1 months for patients without HHF;

• HFrEF with at least one of the following criteria:

• HFrEF with a baseline LDL-C level of less than or equal to 5.9% of total-triglycerides; or

• HFrEF with a baseline HDL-C level of less than or equal to 4.
